Referral forms
Regional urgent suspected cancer referral forms have been developed through close collaboration with the Humber and North Yorkshire Cancer Alliance Clinical Delivery Group, Hull Health Partnership (HHP), York and Scarborough Teaching Hospitals NHS Foundation Trust (YSTHFT), and the regional Local Medical Committees (YORLMC and Humberside LMC’s). Creating these forms at a regional level helps ensure a consistent approach to cancer referrals, with clear and agreed information requirements for clinicians. This standardisation supports high-quality, complete referrals, reduces unwarranted variation, and helps patients access the right diagnostic pathways more quickly and efficiently across Humber and North Yorkshire.
Cancer Incentive Scheme
Cancer Incentive Scheme provides dedicated funding to help 41 Primary Care Networks (PCNs) improve the early diagnosis of cancer. Around £500,000 is being invested across Humber and North Yorkshire to support a wide range of primary care activities focused on earlier detection, increased screening uptake, and improved cancer awareness.
Running from October 2025 to September 2026, the scheme funds protected time for both a GP Clinical Cancer Lead and a Non-Clinical Cancer Lead within each PCN, alongside targeted initiatives to strengthen cancer pathways in primary care.
Funded activities include cancer case audits, proactive case-finding (such as prostate cancer), improved safety-netting processes, quality improvement projects, enhanced use of health checks, better smoking status recording, and actions to increase participation in cancer screening programmes. The scheme also supports workforce development through education, cancer champion training, and shared learning across primary care.
Overall, the scheme aims to reduce pressures on practices while supporting the NHS ambition to diagnose cancers earlier and improve outcomes for patients.

The Network Contract Directed Enhanced Service (DES) is a national contractual framework introduced by NHS England that sits alongside the core general medical services contract for GP practices. Cancer-related information has been sourced from the Network Contract Direct Enhanced Service (DES) 2026/2027 guidance.
